Lead Disclosure Form (Lessor's Disclosure of Information on Lead-Based Paint and/or Lead-Based Paint Hazards )

Under federal law, and if your rental was constructed prior to 1978, this is the disclosure form you use to inform your prospective tenant that you do not know of any lead based paint or other lead based hazard at the property. This form MUST be used along with the pamphlet entitled, “Lead Pamphlet-English (Property Built Prior to 1978”)--Protect Your Family From Lead in Your Home Pamphlet included
