When you have a lease that is expiring, whether it is 30, 45, 60 or even 90 days prior to the actual expiration date (and there is a renewal clause to be exercised by the tenant), this is the form you want to use. It is addressed to all tenants in possession and requires each of their signatures and a timely return of it to the landlord.
