Pest Control Addendum/Acknowledgement KF

Landlords who have hired a pest control company to regularly (periodically) service the rented premises should provide a copy of the disclosure/warning notice prepared by the pest control company to new tenants. Use this form to so that the new tenant can formally acknowledge he or she has received a copy and is aware that a pest control service regularly sprays for insects, rodents and other possible infestations.
