Receipt for Tenant Screening and/or Credit Checking Fee

It's the law in many states, if you require the prospect to pay for the credit report or screening fee, the tenant must agree to it and be provided copy. Even if it is not required in your state, it is always a good idea to use this form to acknowledge receipt of his application and any fee paid.
