To minimize prolonged physical damage and maximize tenant satisfaction, provide this form (3 or 4 with any new tenant documents) to tenants so they have an easy way to ask for repairs. Ever seen a bathroom floor damaged by water from the tub or shower and where the tenant feigned “I didn't know who or how I call for maintenance.” (See periodic inspection forms too).
